Indemnification Clause *

I/we, named below agree to indemnify and hold harmless Ministry Personnel, Evangel Church, and its leaders from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Evangel Church, as well as of any medical treatment authorized by the supervising individuals representing Evangel Church. This consent and authorization are effective only when participating in or travelling to events sponsored by Evangel Church.

Purpose and Extent *

I/We, named below grant permission to Evangel Church to collect and retain this personal information for the purpose of enrolling our child in church programs, assigning our child to the appropriate classes, developing and nurturing ongoing relationships with us and our child, and informing us of program updates and upcoming opportunities at the Church. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel. If you wish Evangel Church to limit the information collected, or to view your child's
information, please contact us.

Communication

A policy is in effect that communication is to be used solely for the dissemination of information. I/we, named below, grant permission for Children/Youth Program Ministry Personnel (staff and volunteers) to communicate with you on behalf of your child via telephone, email, social media, or text.

Parent/Guardian Agreement *

By entering your name below, you agree that you have read, understood and agree with above and sign it to cover all Children/Youth Program activities at Evangel Church on Sundays. A separate Letter of Informed Consent will be sent home for off-site activities and activities of elevated risk.
